Interviewee: Among the programs incorporated into my practice is the chronic disease management program (CDMP), which addresses many chronic conditions such as diabetes. Components of the CDMP related to long-term diabetes patient management include patient surveillance, patient counseling on self-management, initial counseling on lifestyle changes, and other personalized interventions to enhance the discharge of the patient’s life.
