A major challenge related to cardiovascular disease that impacts quality, safety, and financial outcomes in healthcare settings, particularly in my practice setting in Fort Worth, Texas, is the insufficient management of chronic cardiovascular conditions such as hypertension. Cardiovascular disease is the leading cause of death in the United States (Centers for Disease Control and Prevention, 2024). Many residents are at higher risk due to factors like elevated obesity rates, smoking, and limited access to preventive care, which worsen the prevalence and severity of these conditions.

Poorly managed cardiovascular disease leads to worsening patient outcomes, such as an increased risk of heart attacks, strokes, and hospitalizations, especially for patients with hypertension who don’t regularly monitor their blood pressure or follow prescribed medications. Unmanaged or undiagnosed conditions may also result in acute events requiring hospital admissions and critical interventions that could have been prevented with early detection and consistent care.
