Paragraph one: Have you worked in a blame culture or culture of safety? Give specific examples to justify your answer.

             The simple answer is yes. In the past, I worked for a hospital for over ten years. There was a very safe-to-share atmosphere until there were several leadership changes in the last three years, things changed. There was no room for learning or teaching experiences. An example was finally reporting a lab incident that repeatedly happened with another nurse and myself. Initially, I tried speaking with the nurse to educate her on how what she was doing was affecting the patient. After being called into the manager’s office over this issue, and basically being told that I wasn’t her boss and that I was overstepping. Everything was reprimanded and became accusatory.
