Purpose

The purpose of this debriefing is to discover information and deepen knowledge based on the Week 2 Lesson and Shane Peterson i-Human Virtual Patient Encounter to improve skills and clinical reasoning.

Course Outcomes

This debriefing enables the student to meet the following course outcomes:

SOLUTION:

In long term care, I am able to conduct holistic comprehensive assessments.  The depth of assessments vary based upon the findings.  A comprehensive geriatric assessment is conducted to establish and monitor baseline variations in a number of categories.  The assessment’s 5 domains are: physical health, cognitive function, functional ability, psychosocial factors, and socioeconomic factors (Rosen, 2024).  Challenges that our nurses face in this setting include rapid change in condition, frequent reassessment, and inadequate or incomplete reporting from residents.  There are strategies to overcome these challenges.  Documentation of changes in condition and frequent assessments illustrate a clear picture of a resident’s baseline and the deviation.  Different assessments are done on a weekly basis and submitted for review by the care team to alter, if necessary, the plan of care. ……………KINDLY CLICK ON THE LINK BELOW TO ACCESS THE ENTIRE DOCUMENT FOR $10
