Purpose
The purpose of this debriefing is to discover information and deepen knowledge based on the Week 4 Lesson and Ben Bundy i-Human Virtual Patient Encounter to improve skills and clinical reasoning.

SOLUTION:
Conducting cardiovascular and respiratory assessments, especially in a 75-year-old patient with an acute exacerbation of Chronic Obstructive Pulmonary Disease, presents both challenges and learning opportunities. During my assessment of this patient, who arrived with shortness of breath, productive cough, fever, and diminished lung sounds, I encountered the barrier of effectively communicating and understanding the patient’s condition due to their acute distress. The presence of diffuse inspiratory and expiratory wheezing across the lungs underscored the urgency of the situation, making it difficult to perform a thorough assessment while attempting to provide comfort. I learned the importance of maintaining a calm demeanour and explaining each step of the assessment to alleviate patient anxiety. Additionally, I adopted a systematic approach, prioritizing critical findings while remaining attentive to the patient’s non-verbal cues. This experience reinforced the necessity of being adaptable in the clinical setting, allowing me to address unexpected findings, such as the patient’s rapid decline in respiratory function, by quickly collaborating with the healthcare team for timely interventions. As highlighted by McKenzie et al.
