Anna Sink is a 32-year-old female with a full code status, who came in with symptoms of, shortness of breath, chest pain, palpitations, muscle tension, difficulty concentrating, headache, and describes a sense of impending doom, irritability, excessive anxiety, worry and difficulty sleeping.

Brief information related to the clinical background of the situation

 

Anna reports that her symptoms have been going on and off, one to two times a week, for more than six months, with this episode triggered by changes in weather and travel plans. She recalls a past episode of anxiety three years ago with the death of a close family member. Anna has been seeing a therapist on and off for the past three months and states that she practices guided imagery and deep breathing exercises but expresses doubt that these practices help. She lives alone, in a committed relationship and is a community college science teacher. Anna is not currently on any medications, beside an occasional OTC analgesic for her headache. She denies thoughts of harming herself or others. Also denies tobacco or drug use, but occasional wine with friends.
