SOLUTION:

Paragraph one: Unfortunately there were many safety issues noted and they all impacted the patient outcome.  It was really hard to pick one. I do believe the biggest issue or the issue that encompasses a bunch of smaller issues was the lack of management of the postoperative care. The patient should have had follow up on her procedure by an RN or surgeon who was familiar with her care.  When patients are discharged after a procedure, the patient should be given instructions on the signs and symptoms of an infection and who to call if the signs and symptoms are noticed.  All communication should have been documented in her ERS

Paragraph two: I don’t believe technology helped the safety issue identified.
